A license request for a cauldron or thermal machine (as in the designated form) that contains the following data:

A signed admission of ownership illustrating the shares of partners sealed by the Real Estate Registration Department (according to the designated form)
or the company's official founding contract or the Official Gazette where the company's foundation was published

A survey map in the scale of 1:2500 that illustrates the cauldron or thermal machine's location

A geometrical plan in the scale of 1:500 made by a professional non-affiliated syndicate engineer on an original transparency (calc) + 8 copies illustrating the horizontal cross-section of the cauldron or thermal machine's location, the manufacturing room, and the adjoining routes, roads, public utilities, houses within 50 meters in all directions, the distance of each from the cauldron or thermal machine's location. The name of the syndicate engineer, his registration number, and address must be presented with the plan

A copy of the tax card (showing the date on the last tax report presentation) + the original for checking

Social insurance certificate of workers operating the cauldron or thermal machine, attaching the receipt proving up to date payment

As for new cauldrons, a certificate accredited by the Egyptian Organization for Standardization and Quality Control showing that the cauldron have been manufactured according to approved standard specifications, accompanied by the validity certificate issued by the manufacturing party and guarantee against manufacturing deficiencies
